I've found many intermittent problems are caused by loose contact points.
When filing and adjusting contacts try using a small flat screwdriver on the
side of contacts and see if they move around. Use long nose pliers to
squeeze them tight. Black carbon around a contact point is almost a sure
giveaway of a loose point.
